This is where registered team captains can order their team t-shirts for upcoming leagues.  Click the following link to download a copy of the WASA T-Shirt Order Form. 

Follow the instructions located at the bottom of the order form.  Captains MUST complete and return these forms by 9pm on January 7, 2009, to be included in our bulk t-shirt order.  Teams whose forms have NOT been received by the deadline will receive a random team color, and an assortment of sizes ranging from M to XXL.

NOTE:  Your paid base team fee entitles you to a specific number of shirts.  Kickball, Dodgeball, and Ultimate Frisbee teams are entitled to 15 shirts with their base fee.  Flag Football teams are entitled to 10 shirts.  Shirts CANNOT be added once the registration deadline is reached, so even if you have not fully completed your roster, please order the number of shirts you think you will need.  Players above the roster maximum (above 15 for kickball & ultimate, above 10 for flag football) are entitled to a team shirt for their extra fee, so please include these shirts in your order!  Teams who order additional shirts above and beyond what has been paid for will have the appropriate amounts added to their team balance, and will be held responsible for those balances.  For example... a kickball team pays $600, entitling team to 15 shirts, but captain orders 17 shirts.  $80 will be added to the team balance ($40 per player above 15).
